Passports

You will need the following when applying for a U.S. Passport:

• APPLICATION FOR U.S. PASSPORT DS- 11 FORM:                                                                                For your first passport, you must appear in person and complete an APPLICATION FOR U.S. PASSPORT DS- 11 form. It may be completed prior to arrival but must be signed in the presence of the passport agent.  All forms are available at the Newaygo County Register of Deeds Office for free.                                                                                                                                             
• BIRTH CERTIFICATE:                                                                                                                                A certified copy (raised seal) of your Birth Certificate. It is available only from the County Clerk’s Office in the county of your birth or from the State of your birth.                                              Effective April 1, 2011 all Birth Certificates must have parent names.                                                                         
• IDENTIFICATION:                                                                                                                                                A valid Driver’s License, a government issued (Federal, State, Municipal) Identification Card, or a Certificate of Naturalization or Citizenship.                                                                                                  
• PHOTOGRAPH:                                                                                                                              Photographs can be taken in our office location.                                                                              When providing a photograph the photo must have been taken within the past six months       and meet the following requirements:
  • In color
  • Printed on photo quality paper
  • 2 x 2 inches View the Photo Composition Template on the Travel.State.gov website for more size requirement details
  • Taken within the last 6 months to reflect your current appearance
  • Taken in front of a plain white or off-white background
  • Taken with full-face view directly facing the camera
  • With a neutral facial expression and both eyes open
  • Taken in clothing that you normally wear on a daily basis:
    • Uniforms should not be worn in your photo.
    • Do not wear a hat or head covering that obscures the hair or hairline, unless worn daily for a religious purpose. Your full face must be visible, and the head covering must not cast any shadows on your face.
    • If you normally wear a hearing device it may be worn for your photo.
    • As of November 2016 the National Passport Agency will no longer accept passport photos with any type of eyewear including but not limited to prescription glasses.
  • Photo requirements have been changed to require one color photo.                                Note: The Register of Deeds Office can take your passport photo. The fee is $10.00.                                                                                                                                                              
• FEES:
  • Applicants age 16 and over: Must pay $130 for their passport. This payment must be paid by check or money order only payable to the U.S. Department of State. There is a $35 execution fee payable to Newaygo County Register of Deeds by Cash/Check/Money Order/Debit or Credit Card. The passport is valid for 10 years.
  • Applicants age 15 and under: Must pay $100 for their passport. This payment must be paid by check or money order only payable to the U.S. Department of State. There is a $35 execution fee payable to Newaygo County Register of Deeds by Cash/Check/Money Order/Debit or Credit Card. The passport is valid for 5 years.                                   STATEMENT OF CONSENT:  ISSUANCE OF A U.S. PASSPORT TO A CHILD  –               DS- 3053 form. This form should be submitted, along with the Application for U.S. PASSPORT DS- 11 form, for minors under age 16 as evidence of parental permission if:       1) you are a non-applying parent or guardian consenting to passport issuance for your child,   2) or you are an applying parent or guardian, and the written consent of the non-applying parent or guardian cannot be obtained. The Statement of Consent by non-applying parents, for children under the age 16, MUST be Notarized and signed by the non-applying parent and MUST include a photocopy of the front & back of his/her (parent) ID. 
  • All Applicants:  A $35.00 application execution process fee will apply to each application.  This fee can be paid by Cash/Check/Money Order/Debit or Credit Card.  Make payment to the Newaygo County Register of Deeds.                                                                                                                                                                                                                
• APPLICATION SIGNATURE:
  • Until the age of 18, all applicants will need both parents or legal guardians (with legal proof) present to sign your passport application at the time of execution.
  • 16 years of age and above, all applicants will need to sign their own passport application in the presence of the passport agent.
• ALL MINOR CHILDREN APPLICANTS MUST APPEAR IN PERSON.                                                                                

• ARRIVAL OF U.S. PASSPORT:                                                                                                                     You should receive your passport, by mail, within approximately 8 -11 weeks from the receipt of your completed application. If you need your passport in less than approximately 5 -7 weeks, expedited service will be needed for processing the passport. The fee for expedited service is $60 per application, payable by check or money order to the   U.S. Department of State, which is in addition to the regular passport fee.

• Valid for travel by Air, Land and/or Sea.                                                                                                                  
• Valid:  10 years for adults, 5 years for minors                                                                                                        
• Size: 5″ x 3-1/2″ (when unopened)

FEES:  For first time applicants:

• For Adults (16 & Older)
  $130 check/money order payable to: US Dept of State,
  $35 execution process fee payable by cash, check, debit or charge card        

• For Minors (15 & Younger)
  $100 check/money order payable to: US Dept of State,
  $35 execution process fee payable by cash, check, debit or charge card

• Renewal of U.S. Passport Book:  $130 for adults                                                                                (minor passports are not renewable)                                                                                                                                         

• NOT valid for travel by Air.
• Valid for Land and Sea crossings between the U.S.                                                                                    and Canada, Mexico, Bermuda and the Caribbean only.                                                                                   
• Valid: 10 years for adults, 5 years for minors                                                                                                     
• Size: Wallet size (comparable to a Driver’s License or Credit/Debit card size)

• Renewal of U.S. Passport Card : $30 for adults                                                                                      (Minor Passports Are not Renewable)

For first time applicants for both the U.S. Passport Book and U.S. Passport Card:

• For Adults (16 & Older)
  $160 check/money order payable to: US Dept of State
  $35 execution process fee payable by cash, check, debit or charge card                                                                      
• For Minors (15 & Younger)
  $115 check/money order payable to: US Dept of State
  $35 execution process fee payable by cash, check, debit or charge card
