1095-B Forms
Beginning with tax year 2022, due to IRS guidelines, Newaygo County will not be mailing out 1095-B forms for former employees or their beneficiaries who have a Health Care Savings Plan account balance (HCSP). If you would like a copy for your records, please submit in writing a request to Human Resources, PO Box 885, White Cloud, MI 49349.
We will furnish a 1095-B form to anyone eligible upon
request within 30 days of the date the request is received.
1095-C Forms
Beginning with tax year 2024, due to IRS guidelines, Newaygo County will not be mailing out 1095-C forms. If you would like a copy for your records, please submit in writing a request to Human Resources, PO Box 885, White Cloud, MI 49349.
We will furnish a 1095-C form to anyone eligible upon request within 30 days of the date the request is received.
According to the Federal Government’s Transparency in Coverage rule, plans and issuers must make public machine-readable files containing certain pricing information. These machine-readable files include negotiated rates for in-network providers and allowed amounts and historical billed charges for out-of-network providers.
